New Employee Checklist
Use this checklist to guide you through your first few days of employment at UGA

NOTE: Before you proceed with orientation, you should determine if you are benefit-eligible or not. If you're not sure, stop now and ask your supervisor.

You will be asked to provide your first and last name, your Social Security number, and the name of your department. You will also be asked if you are eligible for benefits.

Be aware of benefits enrollment deadlines!
_____ Print & complete employment paperwork:
_____ I-9 Employment Eligibility Verification (complete & turn in this form immediately)
_____ Board of Regents Security Questionnaire & Loyalty Oath (must be notarized)
_____ UGA Intellectual Property Agreement (must be witnessed)
_____ Form W-4 Employee's Withholding Allowance Certificate
_____ Direct Deposit Request Form

_____ When you have completed online orientation and printed all forms, close your Internet browser to accurately record your participation.
_____ In order to receive official campus announcements, you must register your preferred email address. To register your email address, visit https://db.uga.edu/eaddrchk/register.html.
_____ If you are benefit eligible, you should carefully view the Summary of Benefits at the end of the online orientation presentation, which includes benefit plan details and enrollment information. Please plan to attend an optional benefits overview session, held the first & third Mondays of each month from 9:00-10:30am at the Training & Development Center. At these sessions, you may ask questions and receive assistance with enrollment. If the first or third Monday falls on a scheduled holiday (ex., Labor Day), the benefits overview session will be held on the next working day.
_____ Turn in completed benefits paperwork to your departmental secretary, supervisor, or send to Employee Benefits.
_____ Meet with your supervisor to learn more about your department's mission and how you play a role in achieving that mission.
_____ To immediately obtain various campus services, print and complete the New Faculty/Staff Request for Services Form (http://www.busfin.uga.edu/forms/new_employee_ltr.pdf) and follow the instructions on the form.
_____ Meet with appropriate departmental representative for keys, building information, and other administrative policies that relate specifically to your job.
