For 2015, per the University System of Georgia (USG), the monthly tobacco surcharge will increase to $75 and will apply to all health insurance covered employees, retirees, and health-covered dependents (spouse and/or children) age 18 or over.  Be sure to use the MyBenefits@UGA system during open enrollment to indicate your tobacco use! If you fail to indicate your tobacco use during open enrollment (even if you do not use tobacco), you will have a monthly $75 surcharge starting January 2015. Although no surcharge refunds will be permitted, non tobacco users can have the surcharge removed during 2015 by using the the MyBenefits system to indicate they or their family members no longer use tobacco products.

During open enrollment, you may:

Health -Enroll, change plan, add or drop eligible dependents, or cancel coverage.
-May add eligible children below the age of 26, regardless of student or marital status.
-If you are adding eligible dependents, you must provide required documentation by the open enrollment deadline. See the health plan section below for details. Proof of full-time student status for children ages 19-25 is not required.
Dental

-Enroll, add, or drop eligible dependents, or cancel coverage.
-Change from "High" plan to new "Base" Plan.
-If you are adding eligible dependents, you must provide required documentation by the open enrollment deadline. See the dental plan section below for details. Proof of full-time student status for children ages 19-25 is no longer required.

Accidental Death & Dismemberment -Enroll, change, or cancel coverage.
Long Term Disability -Enroll for 2015. May cancel coverage at any time.
Life insurance

-Decrease or cancel coverage for yourself, spouse, or children OR increase coverage for yourself, spouse, or children. Employees may increase optional life insurance coverage by 1x to a maximum of 3x their salary with no statement of health; and up to 8x their salary with a statement of health. All changes should be made using the MyBenefits@UGA system.

Vision insurance

Medical flexible spending account

Adding dependents to your coverage

You must provide supporting documentation in order to add dependents to insurance coverage by the November 14 open enrollment deadline. If you do not provide this documentation, your dependent will NOT be added to your coverage.

Health Insurance

UGA offers four health plans for plan year 2015

  1. Comprehensive Care (formerly the Open Access POS) 
  2. Consumer Choice HSA (formerly the HSA Open Access POS)
  3. BlueChoice HMO 
  4. Kaiser Permanente HMO 

Some 2015 health plan details:

  • All plans have ABA Autism Therapy coverage.
  • Participants in the Consumer Choice HSA will pay 20% coinsurance after the deductible (up from 15%). Deductibles increase in this plan.
  • Participants in the Comprehensive Care plan will have higher deductibles and out-of-pocket maximums (in- and out-of-network).
  • HMO plan participants will have higher co-pays for some services.

  • You are not required to make a healthcare plan election for 2015 unless you wish to change your plan type or your level of healthcare coverage. If you do not select a health plan during open enrollment, you will default to the plan most resembling your current plan, with potential change in cost.

Consumer Choice HSA health plan and HSA employer match money

  • The Consumer Choice HSA plan has very low premiums, high deductibles, and allows the use of health savings accounts (HSAs) — pre-tax savings accounts for eligible medical expenses. HSAs roll over from year to year.
  • The University System of Georgia has contracted with U.S. Bank to provide HSAs for all System institutions. View more information about the U.S. Bank HSA.
  • Participants who are in or enroll in the Consumer Choice HSA plan during open enrollment and contribute to a USG U.S. Bank health savings account (HSA) will receive employer match money in 2015. The employee’s HSA contributions will be matched, dollar for dollar, up to $375 for individual coverage and $750 for more-than-individual coverage. In order for employees to be eligible to receive the match, the employee must make contributions to a U.S. Bank HSA via payroll deduction. Attention departments: funding sources (grants, etc.) for the match may vary.
  • For 2015, the University System of Georgia will again pay the U.S. Bank monthly administrative fees.
  • HSA contribution limits for 2015: individual $3,350, family $6,650, catch-up contribution (age 55 or over) $1000.

Prescription drugs

For 2015, CVS Caremark will be the pharmacy benefit manager (replacing Express Scripts) for all health plans except the Kaiser Permanente HMO. The CVS Caremark network is larger and includes dozens of pharmacies in addition to CVS. Learn more about CVS Caremark for 2015 and locate in-network pharmacies.

Dental Insurance

The University System of Georgia (USG) has again contracted with Delta Dental for 2015. Dental premiums will increase by 12% in 2015. The annual benefit maximum for the High plan (see choices below) will remain $1,500 for 2015. The benefit maximum for the Base plan will be $1,000 per year. You may cover eligible family members; however, if you are adding new dependents, you may be requred to provide a birth certificate.

TWO DENTAL PLAN CHOICES:
You may choose the High plan or the Base dental plan. The Base plan has lower premiums and coverage. Enroll or change your coverage using the MyBenefits@UGA system.

Life Insurance

The University System of Georgia (USG) has again contracted with Minnesota Life for 2015 basic and supplemental life insurance. You are encouraged to enter your beneficiary designations in the MyBenefits@UGA system. Employees may increase optional life insurance coverage by 1x to a maximum of 3x their salary with no statement of health; and up to 8x their salary with a statement of health. Flexible Spending Accounts

The vendor for all 2015 FSAs is U.S. Bank. Depending upon the health plan you select, you have choices regarding spending accounts. Review your spending account choices.

The University System of Georgia has contracted with U.S. Bank to administer the flexible spending account program (FSA) for health and dependent care reimbursements. Using U.S. Bank you'll enjoy features, including:

  • 24/7 online access to your account information
  • Debit spending card (card fee paid by the University)
  • An additional 2 ½ month grace period to incur expenses and submit claims
  • Toll-free customer service phone center Monday through Friday, excluding holidays, from 8:00am to 8:00pm (ET) at 877-470-1762.

The dependent care spending account allows employees to pay for eligible dependent care expenses with before-tax dollars by allowing pre-tax contributions to a flexible spending account. Contribution limit for 2015: $5,000.

The health flexible spending account allows employees to pay for eligible health care expenses such as co-pays, deductibles, co-insurance, and medications. Contribution limit for 2015: $2,500.

The limited purpose flexible spending account is available only to contributors to 2015 US Bank health savings accounts. The LPFSA is for dental and vision care expenses only and coordinates with the HSA. Contribution limit for 2015: $2,500.

Vision Insurance

EyeMed will continue to provide vision insurance for USG institutions. If you choose to buy vision insurance, it will supplement any vision benefits included in your health plan, if applicable. Review the EyeMed vision insurance.  Find an EyeMed provider (select the "Insight" network and type in your zip code). Enroll using the MyBenefits@UGA system. You may cover eligible family members; however, if you are adding new dependents, you may be requred to provide a birth certificate.

Short-Term Disability Insurance

MetLife will again provide STD insurance for 2015. Short-term disability insurance provides 60% salary replacement if you cannot work due to illness or injury for up to 12 weeks.

Enroll using the MyBenefits@UGA system.

The cost of the MetLife STD insurance is based on 60% of your weekly benefit amount. The rate remains the same for 2015: $.291/$10 of covered benefit. For example, if your gross weekly salary is $600, your monthly premium would be (60 x .60) x .224 = $8.06.

You will begin receiving income benefits once you are out of work for more than 14 days.

Long-Term Disability Insurance (LTD)

MetLife will again provide LTD insurance for 2015. Long-term disability insurance provides 60% salary replacement in the event you are unable to work for at least 90 days due to serious illness or injury. The cost of the plan is based on your salary. The rate remains the same for 2015: .224%/$100 of covered salary. Enroll using the MyBenefits@UGA system.

For example, if your gross monthly salary is $2500, your monthly premium would be 25 x .224% or only $5.60 per month. Per LTD plan rules, individuals with pre-existing conditions are not allowed to file claims for that condition for 365 days.

If no action is taken during open enrollment, individuals who already have LTD coverage will continue their present level of coverage for 2015.

Accidental Death & Dismemberment Insurance (AD&D)

The University System of Georgia has again contracted with Minnesota Life to provide AD&D coverage (in addition to the AD&D insurance included with your life insurance). During open enrollment, you may enroll in the AD&D plan with no statement of health required. You may also change your coverage amount, change your level of coverage (family vs. individual), change your beneficiary, or cancel your coverage.

You should enroll by using the MyBenefits@UGA system. If no action is taken during open enrollment, individuals who already have AD&D coverage will be automatically enrolled with comparable coverage for 2015.
