The health, dental,
health care flexible spending account, and dependent care flexible spending account elections you request
during your initial enrollment will remain in effect
for the entire plan (calendar) year. You may change
the level of coverage (employee only, employee + spouse, employee + one child, or family) or cancel your participation
in the plans only if you experience an IRS-approved
change
in family status.
If you experience
a family status change, visit the HRconnection Web site within 31 days of the event and select the "Change coverage" tab. Select the reason for the change and complete the remainder of the online form. When you press "Send," this information will be forwarded to UGA HR. You will also be required
to furnish proof of the change in family status.
If you do not request changes in your coverage within 31 days of the qualifying event, you must wait until open enrollment to request coverage changes. If your dependents become ineligible for coverage, you must notify UGA Human Resources within 31 days of the qualifying event. If you fail to do this, your ineligible dependents will not be covered, but you will pay higher premiums until you can make a change during open enrollment. You will not receive a refund of premiums for any ineligible dependents you fail to remove from your coverage.
